By checking the box below, I, the undersigned participant, acknowledge, and agree to the following terms and conditions for my participation in events organized by Mo Dot LLC, specifically held at Waterloo Pickleball facilities:

Voluntary Participation: I am fully aware that participating in events organized by Mo Dot LLC is voluntary and acknowledge that such participation may involve inherent risks and potential hazards.

Assumption of Risk: I willingly and knowingly assume full responsibility for all risks and dangers associated with my participation, including, but not limited to, the risk of physical injury, loss, or damage, whether those risks arise from the negligence of Mo Dot LLC, its associates, or from other participants.

Release of Liability: I hereby release, waive, discharge, and covenant not to sue Mo Dot LLC, its directors, employees, volunteers, agents, and the owners or lessors of the premises where the event occurs (specifically Waterloo Pickleball), from any and all liability, claims, demands, actions, and causes of action whatsoever directly or indirectly arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in such activity, or while on the premises where the event is conducted.

Medical Treatment Consent: In the event of an injury or illness during my participation, I consent to receive medical treatment deemed necessary by event organizers or medical personnel. I accept full responsibility for the costs related to such medical treatment.

Media Release: I grant Mo Dot LLC and its legal successors the irrevocable and unrestricted right to use and publish photographs or videos of me, or in which I may be included, for any lawful purpose, including but not limited to promotional and advertising purposes, in any medium now known or hereafter developed, without seeking further permission or providing compensation to me.

All Sales Are Final: I understand and agree that all ticket sales are final, and no refunds will be issued under any circumstances, including but not limited to changes in personal circumstances, medical issues, or event cancellation/weather events.

I affirm that I have read this Release of Liability and Media Consent document, fully understand its terms, and acknowledge that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my acknowledgment for this to be a complete and unconditional release of all liability to the greatest extent allowed by law.
